Required Forms for the 2024-2025 School Year 

In preparation for the 2024-2025 school year, below is required health information that will be needed to start school in the fall. If you have questions regarding required immunizations, please refer to the VDH website.

Health Forms Required for NEW STUDENTS, and ALL KINDERGARTEN STUDENTS  ONLY: New students in grades preschool-8th grade as well as EVERY rising KINDERGARTEN student (including those currently registered in St. John’s PreK) are required to submit a Physical form completed in the last 12 months to the health office (dated 8/23/2023 or later) along with an UPDATED IMMUNIZATION RECORD. The Commonwealth of Virginia School Entrance Health Form 2024 can be uploaded to the “Health History Update” under the medical tab on your student’s online enrollment account or emailed to the health office at, [email protected] no later than August 22, 2024.

Rising 7th Grade Requirement: Effective July 1, 2021, all rising 7th graders must have documented that they received one dose of the Meningococcal ACWY (MenACWY) vaccine. All 7th graders must also have documented that they received one dose of the Tdap vaccine after the age of 7.

Health Forms Required for RETURNING STUDENTS ONLY: If your child was enrolled at SJBA last year and is not entering Kindergarten, all that is required is to fill out the Confidential Health History form and upload it to your student(s) online enrollment account as well as the submission of any updates to the student’s immunizations records to the school nurse. Kindly submit this form no later than August 22, 2024.

If you are opting out of vaccination requirements because of religious beliefs, the health office will need to have a religious exemption form on file. This form can be found here: https://www.doe.virginia.gov/support/health_medical/certificate_religious_exemption.pdf.

Additional Forms Required for All NEW and RETURNING STUDENTS:

The following forms are required at the beginning of each school year for all new and returning students. Please print out and complete the following forms.

One Form Per Family:

1. Dismissal-Sheet (please return to the front office)

One Form Per Student:

1. Permission for Emergency Care (on the enrollment site)
2. Media Release (on the enrollment site)
3. Medication Authorization Form 2024-2025 (if applicable)
4. Inhaled Medication or Nebulizer Treatment Authorization  and the Virginia Pediatric Asthma Action Plan -2024 (if applicable)
5. Diabetes Reference and Indemnification (if applicable)
6. Seizure Action Plan (if applicable)
7. Food Allergy and Anaphylaxis Care Plan and Carry Epi Agreement (if applicable)

These forms and any medications are due August 22, 2024.

Students in Grades 5-8 that plan on playing on an SJBA sports team will need a sports physical on file prior to the start of the season. The Diocese uses the Virginia High School League sports physical forms–skip any fields that apply to high school students.
